Authorized agent should notify provider to begin the renewal process two months prior to expiration of their license.

 

A license will not be issued if a provider fails to submit all required licensing documentation prior to the expiration date. The regional office should contact the central office if special circumstances apply that would require a different effective date.

 

All required documentation as identified on check list must be received at this time.
